At Lighthouse Therapy Center, we accept a wide range of insurance plans. If you can’t see your provider on the list, get in touch and we’ll be happy to give you more information or to make special accommodations if you are not adequately insured.

• Physician referral
• Parent/Guardian photo ID
• Insurance card (front & back)
• Most recent evaluation/Plan of Care and discharge summary, if your child received therapy elsewhere
